2021–2022 Student Marital Status Verification

Financial Aid and Scholarship Services will request this documentation only when needed to complete verification. If this item is listed under your Financing Tasks in My ASU, your financial aid awarding and disbursement will be placed on hold.

You can view your document status on My ASU’s Finances tab. Please allow 2 to 3 business days for receipt to show in your Financing Tasks.

Purpose: This form is used for two different scenarios:

  1. The marital status reported on your FAFSA conflicts with the information reported on your household size form, your tax return or other documents submitted in support of your financial aid application.
  2. You are a dependent student and you got married after you completed the 2020-2021 FAFSA.

Print the IRS Form 4506-T from the IRS website. Complete lines 1 through 4 and enter the student’s 10-digit ASU ID number on line 5 (customer file number). Be sure to check box 7 (Verification of Nonfiling). On line 9, enter the date 12/31/2019. Then, sign and submit to the IRS. It will take 7 to 10 business days to receive by mail.

Print the IRS Form 4506-T from the IRS website. Complete lines 1 through 4 and enter the student’s 10-digit ASU ID number on line 5 (customer file number). Be sure to check box 8 (Form W-2). On line 9, enter the date 12/31/2019. Then, sign and submit to the IRS. It will take 7 to 10 business days to receive by mail.

Tax Return Transcript

Print the IRS Form 4506T-EZ from the IRS website. Complete lines 1 through 4 and enter the student’s 10-digit ASU ID number on line 5 (i.e., customer file number). On line 6, enter the year 2019. Then, sign and submit to the IRS. It will take 7 to 10 days to receive by mail.
