2021–2022 Parent Amended Tax Return Verification

Financial Aid and Scholarship Services will request this documentation only if it is needed to complete your financial aid file. If this item is listed under your Financing Tasks in My ASU, your financial aid awarding and disbursement will be placed on hold.

You can view your document status on My ASU’s Finances tab. Please allow 2 to 3 business days for receipt to show in your Financing Tasks.

Purpose: To complete your financial aid file, your parent's amended 2019 tax return must be verified.

This form must be completed by your parent on the FAFSA. You will need to provide the parent listed on your FAFSA guest access to My ASU in order for them to electronically complete and sign parent forms. To provide guest access, complete the Parent Guest Access checklist, if you have not already done so. Once you provide guest access, your parent will need to complete the Parent Signature Verification checklist in order to begin to sign and complete forms electronically.

The required documentation depends on whether or not your parent(s) successfully used the IRS Data Retrieval Tool on the 2021–2022 FAFSA and who initiated the change to their 2019 tax return (i.e., parent, the IRS or ASU).

Scenario A: Your parent successfully used the IRS DRT on the 2021–2022 FAFSA and your parent amended their 2019 tax return. Submit one of the following:

  1. A signed copy of your parent's 2019 Amended Tax Return (i.e., 1040X).
  2. A copy of your parent's 2019 Record of Account Transcript.

Scenario B: Your parent did not use the IRS DRT on the 2021–2022 FAFSA and your parent amended their 2019 tax return. Submit one of the following:

  1. A copy of your parent's 2019 tax return and a signed copy of your parent's 2019 Amended Tax Return (i.e., 1040X).
  2. A copy of your parent's 2019 tax return and a copy of their 2019 Record of Account Transcript.

Scenario C: ASU identified an error on your parent's 2019 tax return that must be amended with the IRS. Submit one of the following:

  1. A signed copy of your parent's 2019 Amended Tax Return (i.e., 1040X).
  2. A copy of your parent's 2019 Record of Account Transcript.

Scenario D: The IRS amended your parent's 2019 tax return. Submit both of the following:

  1. Documentation that includes the changes made by the IRS (i.e., CP2000 Notice, CP3219A Notice).
  2. A copy of your parent's 2019 tax return OR your parent's successfully used the IRS DRT on the 2021–2022 FAFSA.

The U.S. Department of Education does not require financial aid administrators to have special knowledge or expertise regarding the U.S. tax code; however, FAAs are required to have basic knowledge of those tax issues that could constitute conflicting information and substantially affect the expected family contribution.

FAAs are obligated to know:

  • If someone is required to file a tax return.
  • The correct filing status for a tax filer (i.e., single, head of household, married filing jointly, married filing separately, widowed).
  • Someone cannot be claimed as an exemption by more than one person.

All federal student aid recipients, their spouses (if married), and their parents (if dependent) are expected to comply with published tax laws. When conflicting information is found, it must be resolved before the financial aid file can be completed and processed.

There may be rare cases when an individual did not file a 1040X Amended Tax Return with the IRS but had their tax and income information changed by the IRS.

This may be due to a tax filer's mathematical mistakes or when the IRS received information from a third party (such as an employer or financial institution) that doesn't match the information the tax filer reported on their tax return. Affected tax filers are sent an IRS notice or letter by mail indicating the changes.

If the tax filer no longer have a copy of their IRS notice or letter, contact the IRS at 1-800-829-1040 to request a duplicate or submit one of the following document combinations to ASU Financial Aid and Scholarship Services.

  • A 2019 tax return and a 2019 Record of Account Transcript.
  • A 2019 Tax Account Transcript with a 2019 tax return, regardless of DRT usage.

There are several ways to request a Tax Return Transcript from the IRS.

1. Your parent can instantly retrieve their online TRT at the IRS Get Transcript website and clicking the Get Transcript Online button. The online tool requires the user to register before submitting a transcript request. The user must have the following:

  • Access to a valid email address.
  • A Social Security number.
  • Their filing status and address from the last-filed tax return.
  • A text-enabled mobile phone in the user’s name (pay-as-you-go plans cannot be used).
  • Their personal account number from one of the following: credit card, home mortgage loan, home equity line of credit or car loan.

If the tax filer has a credit security freeze with Equifax, they must request a temporary freeze lift until they have successfully registered or the IRS will not be able to verify their identity. They will need to contact Equifax to resume the freeze unless they scheduled it to resume automatically. For more information, visit IRS Get Transcript FAQs .

2. Have your parent request their TRT to be mailed to them by visiting the IRS Get Transcript website and clicking Get Transcript by Mail.

3. Have your parent call 1-800-908-9946 and use the IRS automated phone system to request their TRT to be mailed to them.

4. Submit an IRS Form 4506T-EZ Short Form Request for Individual Tax Return Transcript to the IRS by printing the form at www.irs.gov. Complete lines 1 through 4. Enter your 10-digit ASU ID number on line 5 (customer file number). Enter the tax year 2019 on line 6. Finally, sign and submit to the IRS.

This document becomes available 8 to 12 weeks after the IRS receives your 1040X. When available, there are two ways to request an IRS Record of Account Transcript from the IRS.

1. Your parent can instantly retrieve a Record of Account Transcript online by visiting the IRS Get Transcript website and clicking the Get Transcript Online button. The online tool requires the user to register before submitting a transcript request. The user must have the following:

  • Access to a valid email address.
  • A Social Security number.
  • Their filing status and address from the last-filed tax return.

2. Submit the IRS Form 4506-T Request for Transcript of Tax Return to the IRS by printing the form at www.irs.gov. Have your parent complete lines 1 through 4 and enter your 10-digit ASU ID number on line 5 (customer file number). On line 6, enter 1040 and mark box 6c (Record of Account). On line 9, enter 12/31/2019. Finally, have your parent sign and submit to the IRS.
