Cigna Corporate Eligibility Confirmation

Eligible Cigna employees attending Arizona State University's online campus should submit an e-Letter of Eligibility to receive the ASU Cigna Scholarship. Please allow 3-5 business days for processing once a form is submitted. Important Sponsored Billing Information: While Arizona State Financial Aid and Scholarship Services requires an eligibility confirmation letter each semester to post scholarships this process is separate from sponsored billing requirements. Students interested in using sponsored billing services should work with Student Business Services.

Instructions:

1. Please access the Powerform below and complete your name and email to access the full form. We recommend, wherever possible, that you complete the form using your Cigna email address to expedite the validation of your employment.
2. Do not submit this form if you are morethan 30 days prior to the start of the semester. Forms submitted more than 30 days prior to the start of the term will be returned.
3. Letters will be accepted throughout the semester, but awards will not be issued after the last day of classes for a given term.
4. If you do not have an ASUrite, please wait until an ASUrite has been created to submit your letter.
