If you have a task that is required to complete the verification process but your parent(s) does not have a social security number to electronically sign their form or your parent(s) have chosen not to electronically sign the form, the PDF form can be submitted here.
Please be aware that information regarding what to submit may have been sent to your ASU email account.
The student must upload the form from their account in order to be linked to their account. If the parent uploads the document to this page, the form will not be reviewed.
