2024-2025 Parent amended tax return verification

If this item is listed under your Financing Tasks in My ASU, your financial aid disbursement will be placed on hold until you provide the information requested.

Why this is needed

To complete your financial aid file, your parent's amended 2022 tax return must be verified.

This eForm must be completed by your parent listed on the FAFSA. You will need to provide this parent with guest access to My ASU in order for them to electronically complete and sign parent forms. To grant your parent guest access, visit My ASU - Add a Guest. Under Finances, check the box next to View my tasks. Once you provide guest access, your parent will need to complete the Parent Signature Verification task in order to sign and complete forms electronically.

ASU identified an error on your parent's 2022 tax return that must be amended with the IRS. Submit one of the following:

  1. A signed copy of your parent's 2022 2-page amended tax return (i.e., 1040X).
  2. A copy of your parent's 2022 IRS Record of Account Transcript.

The U.S. Department of Education does not require financial aid administrators to have special knowledge or expertise regarding the U.S. tax code; however, financial aid administrators are required to have basic knowledge of those tax issues that could constitute conflicting information and substantially affect the student aid index (SAI).

Financial aid administrators are obligated to know:

  • If someone is required to file a tax return.
  • The correct filing status for a tax filer (i.e., single, head of household, married filing jointly, married filing separately, widowed).

All federal student aid recipients, their spouses (if married), and their parents (if dependent) are expected to comply with published tax laws. When conflicting information is found, it must be resolved before the financial aid file can be completed and processed.

There are several ways to request a tax return transcript from the IRS.

  1. Your parent can instantly retrieve their online Tax Return Transcript (TRT) on the IRS Get Transcript website by clicking the Get Transcript Online button. The online tool requires the user to register before submitting a transcript request. The user must have the following:
    • Access to a valid email address.
    • A Social Security number.
    • Their filing status and address from the last-filed tax return.
    • A text-enabled mobile phone in the user’s name (pay-as-you-go plans cannot be used).
    • Their personal account number from one of the following: credit card, home mortgage loan, home equity line of credit or car loan.
    During the registration process, the IRS will send the user a confirmation code via email and an authentication code via text. The IRS email and text will contain a one-time code that the user will use to finalize your registration. After a login has been created, continue by entering your IRS username. Select the reason the transcript is needed: Higher education or student aid. Go to the wage and income transcript section and click the 2022 year to print your transcript.
  2. If the tax filer has a credit security freeze with Equifax, they must request a temporary freeze lift until they have successfully registered, or the IRS will not be able to verify their identity. They will need to contact Equifax to resume the freeze unless they scheduled it to resume automatically. For more information, visit IRS Get Transcript FAQs.
  3. Have your parent request their TRT to be mailed to them by visiting the IRS Get Transcript website and clicking Get Transcript by Mail.
  4. Have your parent call 1-800-908-9946 and use the IRS automated phone system to request their TRT to be mailed to them.
  5. Submit an IRS Form 4506T-EZ Short Form Request for Individual Tax Return Transcript to the IRS by printing the form at www.irs.gov.
    • Complete lines 1 through 4.
    • Enter your 10-digit ASU ID number on line 5 (customer file number).
    • Requesting a Tax Return Transcript: Enter the tax year 2022 on line 6. Finally, sign and submit to the IRS.
    • Requesting a Record of Account Transcript: On line 6, enter 1040 and mark box 6c (Record of Account). On line 9, enter 12/31/2022. Finally, sign and submit to the IRS.

You can view your document status on My ASU under the Finances tab. Please allow two to three business days after you submit the form for our receipt to show in your Financing Tasks.
