For use by ASU personnel only. Departments can complete this form to request information or a query from the ASU Scholarship Office. Please complete the form and send as an attachment to SFAODATA@ASU.EDU. Please allow at least 4 - 6 weeks for requests to be processed. Peak business times require additional processing time.
