Report Suspected Identity Theft or Enrollment Fraud at ASU

Use this form to report suspected identity theft or fraud involving an application to or enrollment at Arizona State University (ASU). ASU will use this report to investigate whether an account exists and to flag it as potentially fraudulent.

If you believe your identity has been stolen, we recommend completing these steps as soon as possible:

  1. File a police report with your local law enforcement agency. Keep a copy for your records.
  2. Submit a report to the Federal Trade Commission (FTC) at identitytheft.gov or call 1-877-IDTHEFT (1-877-438-4338).
  3. Report financial aid fraud to the U.S. Department of Education, Office of Inspector General:

    2. Online: Submit a complaint
  4. Contact the three major credit bureaus to place a fraud alert or credit freeze:
    1. TransUnion
    2. Equifax
    3. Experian
