If the you (the parent) are a former ASU student then a copy of this form will be submitted to the ASU University Registrar's Office. This form is not used to change a current student’s information.

Financial Aid and Scholarship Services will request this form only if it is needed to complete your financial aid file. This item holds the disbursement of your private education loan if it is listed on your My Tasks on My ASU.
You must complete this form if you have requested a private loan from your lender and you have unaccepted Federal Direct Loans that were offered to you.
Per federal regulations, when a student withdraws from a university, the school must offer any post withdrawal disbursement of loan funds of any Federal Direct Loan a student earned for the term prior to their withdrawal. The amount earned is determined as part of a Federal Return of Title IV Funds calculation.

Your parent checked Yes to the default loan question on the Federal Direct PLUS Loan application. In order to continue processing your Federal Direct PLUS loan, your parent will need to complete the Parent PLUS default form.
Submit this form if for any reason you (the parent) is ineligible to apply for the Federal Direct Parent PLUS loan. Submission of this form does not guarantee additional funding. Parent’s refusal to take out a Parent PLUS loan is not considered a special circumstance.
This form is for parents to use in the event they are unable to accept, or are having difficulties accepting, the Parent PLUS Loan online at www.asu.edu/go/parentloan.
This form can also be used to adjust the amount of the Parent PLUS Loan after the loan has already been accepted.
Financial Aid and Scholarship Services will request this eForm only if it is needed to complete your financial aid file. The disbursement of your financial aid is on hold if this is listed on your task list on My ASU.
