Amazon Career Choice Eligibility Confirmation

Eligible Amazon Career Choice employees who have reached their annual Payment Request maximum can still receive the ASU Amazon Career Choice scholarship.  Eligible Career Choice employees should submit proof of eligibility (see instructions here) via the link below.  Please allow 3-5 business days for processing once eligibility has been submitted. Instructions:

1. Please access the e-form below to attach your proof of eligibility.
2. Proof submitted should clearly show eligibility date.
3. Proof of eligibility must be submitted within the first seven days of the session following the requirements for Amazon Career Choice program.
4. If you do not have an ASUrite, please wait until an ASUrite has been created to submit your letter.
