2024-2025 Contributor Social Security number verification
If this item is listed under your Financing Tasks in My ASU, your financial aid disbursement will be placed on hold until you provide the information requested.
Why this is needed
The same Social Security number (SSN) was reported for both you and one of your contributors (student’s spouse, parent, or parent’s spouse/partner) on the FAFSA. ASU's Financial Aid and Scholarship Services will need to verify the correct SSN for your contributor in order to continue processing your financial aid file.
Important
If your contributor does not have a copy, or has lost their Social Security card, they can request a copy from the Social Security Administration online at ssa.gov or by calling 1-800-772-1213.
If your contributor does not have an SSN, they will contact the Federal Student Aid information Center (FSAIC) at 1-800-433-3243 and will be required to complete a knowledge-based identity verification process with TransUnion® to verify their identity and provide login credentials.
Instructions
- Click on the eForm below and follow the instructions provided.
- Upload a copy of your contributor's Social Security card.
- Submit the eForm.
You can view your document status on My ASU's Finances tab. Please allow two to three business days after you submit this request for our receipt to show in your Financing Tasks.
