2025-2026 Parent Federal Loan Discharged Due To Disability
If this item is listed under your Financing Tasks in My ASU, your financial aid disbursement will be placed on hold until you provide the information requested.
Why this is needed
Federal Student Aid (FSA) records show that your parent has one or more federal student loan(s) that were discharged due to a total and permanent disability (TPD). In order for Financial Aid and Scholarship Services to continue processing your financial aid, we need your parent to confirm if they want additional federal parent PLUS loan(s).
This eForm must be completed by one of the parents listed on the FAFSA. You will need to provide the parent guest access on My ASU in order for them to electronically complete and sign parent forms.
To grant your parent guest access, visit My ASU - Add a Guest. Under Finances, check the box next to View my tasks. Once you provide guest access, your parent will need to complete the Parent Signature Verification task in order to sign and complete eForm electronically. Important
Though this form pertains to seeking a new parent PLUS loan, if your parent has other loans other than parent PLUS loan in TPD, they may be affected.
If your parent’s disability discharge was approved based on a Social Security Administration (SSA) or a medical professional’s certification, they are subject to a three-year post-discharge monitoring period beginning on the date the TPD discharge is granted. If your parent chooses to receive new federal parent PLUS loans during this time, their previous discharge will be reinstated.
This means:
- Your parent’s loans or TEACH grants will no longer be in discharged status.
- Your parent will need to repay their previously discharged loans.
- Your parent will need to complete TEACH grant service obligations again.
Additional requirement for Veterans Affairs (VA) disability discharges
If your parent’s loan discharge was based on a determination from the U.S. Department of Veterans Affairs (VA), additional documentation may be required before new federal parent PLUS loans can be processed.
Your parent must either:
- Provide a physician’s statement confirming their ability to engage in substantial gainful activity and the Form listed below, or
- Ensure their federal loan status has been fully updated in the National Student Loan Data System (NSLDS) to reflect that the VA discharge has been reversed.
Documentation from a loan holder alone is not sufficient. If these requirements are not met, your parent will not be eligible to receive new parent plus loans.
Before moving forward, have your parent contact the Department of Education’s TPD Servicer, Nelnet at 1-888-303-7818 to check their current status and let them know their plan to receive new federal aid - parent PLUS loan.
Instructions
If your parent does not wish to be considered for federal parent PLUS loans:
- Click the link provided in their My ASU task to access the eForm.
- Select that they do not want to be considered.
- Submit the completed eForm.
If your parent wishes to be considered for federal parent PLUS loans and this is their first request for awards since their loans were discharged due to disability:
- Indicate they wish to be considered and whether or not their disability discharge was determined by the Veterans Affairs Office.
- Acknowledge each statement (type their full initials).
- Upload a physician's letter or download the ASU Physician Statement at the bottom of this page only if they are requesting new federal parent PLUS loans.
- Upload a current (within 30 days) letter from TPD Servicer indicating their discharged loans have been reinstated and repayment has resumed if they are subject to the post-discharge monitoring period. If they are unsure whether they are in the post-discharge monitoring period, they can contact the TPD Servicer Nelnet at (888)-303-7818.
- Submit the completed eForm.
If they’ve already provided a physician’s letter and are requesting continuing funds:
- Click the link provided in their My ASU task to access the eForm.
- Indicate they wish to be considered and whether or not their disability discharge was determined by the Veterans Affairs Office.
- Acknowledge each statement (type their full initials).
- Upload a current (within 30 days) letter from TPD Servicer indicating their discharged loans have been reinstated and repayment has resumed if they are subject to the post-discharge monitoring period. If they are unsure whether they are in the post-discharge monitoring period, they can contact the TPD Servicer Nelnet at (888)-303-7818.
- Submit the completed eForm.
You can view your document status on My ASU's Finances tab. Please allow two to three business days after you submit it for our receipt to show in your Financing Tasks.
