You must complete this eForm if you have requested a private loan from your lender and you have unaccepted Federal Direct Loans that were offered to you.
Use this form to update Parent PLUS Loan borrower information for Financial Aid and Scholarship Services. If you (the parent) are a former ASU student then a copy of this form will be submitted to the ASU University Registrar's Office. This form is not used to change a current student’s information.
Submit this form if for any reason you (the parent) is ineligible to apply for the Federal Direct Parent PLUS loan. Submission of this form does not guarantee additional funding. Parent’s refusal to take out a Parent PLUS loan is not considered a special circumstance.

This form can also be used to adjust the amount of the Parent PLUS Loan after the loan has already been accepted.
Per federal regulations, when a student withdraws from a university, the school must offer any post withdrawal disbursement of loan funds of any Federal Direct Loan a student earned for the term prior to their withdrawal. The amount earned is determined as part of a Federal Return of Title IV Funds calculation.

If no response is received within the date indicated on the Post Withdrawal email you previously received, the loans will be cancelled.
